Jack Blake was born at Brighton, Sussex on 19 December 1893 and commenced his naval service as Boy 2nd Class in September 1911, in H.M.S. Ganges II. His medals for service in the Great War were issued to H.M.S. Diligence, and his War Gratuity was paid to H.M.S. Columbo. He was awarded his Long Service and Good Conduct Medal in March 1927.
