D.C.M. London Gazette 1 January 1818; citation 17 April 1918:

‘For conspicuous gallantry and devotion to duty. He led forward a party to bring in a man who had been seriously wounded. While they were carrying him in the rest of the party were wounded by shell. Although badly shaken, he dressed their wounds, being under shell fire the whole time.’

Surname corrected to ‘Periam’ on D.C.M. Card and confirmed as such on Medal Index Card. Also entitled to British War and Victory medals.
