John Thomas Smith was born and lived in Church Gresley, Derbyshire and enlisted at Lichfield, Staffordshire. As a Serjeant in the 1st Battalion North Staffordshire Regiment he entered the France/Flanders theatre of war on 10 September 1914. He was killed in action on 12 July 1916. With copied m.i.c. - clasp confirmed.
